Meet Millie: A New Model for Maternity and Gynecology Care

Care Model: Midwifery-Led, OB-Supported

At the center of Millie's care model are Certified Nurse Midwives (CNMs), who serve as the primary clinicians for the majority of patients. CNMs are advanced practice clinicians who complete graduate-level nursing programs and are nationally board certified by the American Midwifery Certification Board. They specialize in managing low- to moderate-risk pregnancies, physiologic labor, birth, and non-surgical gynecology.

When a pregnancy becomes higher risk or requires additional management, care is escalated to a Maternal-Fetal Medicine (MFM) specialist or OB-GYN. Millie's OB-GYN is available for consults, complicated gynecology management, and procedures, and hospitalist OB-GYNs serve as backup for inpatient care. This tiered structure allows most patients to receive personalized, relationship-based care from a midwife while ensuring specialist support is available whenever clinical needs increase.

Births take place at Good Samaritan Hospital in San Jose and Alta Bates Hospital in Berkeley, combining the continuity of midwifery care with the safety net of a hospital setting.

Care can begin as early as a positive pregnancy test, with both virtual and in-person visit options available — giving patients flexibility in how and when they connect with their care team.

Beyond pregnancy, Millie offers comprehensive gynecology services for patients at any life stage, including:

  • Annual exams and pap smears
  • Birth control counseling and management
  • Menopause care
  • PCOS and other hormonal concerns
  • Pelvic and vulvar pain
  • Sexual health
  • Infections and urinary concerns
  • Breast and chest health
  • Abnormal bleeding

Both in-person and virtual visits are available for gynecologic care, and more complex gynecology needs are managed directly by Millie's OB-GYN.

Wraparound Support

Millie's approach extends beyond clinical visits. Every patient is paired with a Millie Guide — a practicing doula trained to provide educational and emotional support throughout pregnancy and postpartum. Millie Guides help patients think through topics like birth preferences and postpartum planning, and they're accessible through the Millie app alongside the rest of the care team.

Patients also have access to an integrated network of specialists, including lactation consultants, nutritionists, and therapists, as well as classes, workshops, and support groups. The Millie app rounds out the experience with 1:1 messaging and timely, educational resources delivered throughout a patient's care journey.
